Judith helps patients and their carers/relatives to find information, support and advice.  Typically, this might be at a time when your situation has changed and you are finding it more difficult to cope.

If your question is not directly medical, but is impacting on your life, Judith is the person to speak to.

She also co-ordinates Ludlow Compassionate Communities, which is a group of volunteers associated with Portcullis GP Surgery. The volunteers befriend patients of the practice.

Judith is available by appointment and for telephone advice.

What is a Community and Care Coordinator?

A Community and Care Coordinator is based in your local GP Practice. They assist patients across all age groups in need of help, support and advice by signposting them to other useful services and activities. Community and Care Coordinators work with the NHS, the Council and Voluntary Services and have become local experts on what’s happening in your community. They can help you to keep socially active and maintain your independence.
