Accessible Information Standardlogo

The Accessible Information Standard aims to ensure that patients (or their carers) who have a disability or sensory loss have access to information they can understand, and any communication support they might need.

Should you have any requirements or know of a patient who needs support, please let the Practice know and we will help provide support whether that is providing information in large print or putting a patient in touch with British Sign Language (BSL) interpreter, email or Braille.

We will be collecting this information initially for new patients on registering and for patients who are known to the practice to have a disability which requires communication via a specific method.

If you require a specific method of communication because of a disability or sensory loss, please inform the practice as soon as possible.

You can download this form and post/hand it into the Surgery:
